There is a $50 deductible for an accidental injury which requires an Emergency Room visit.

Per Accident Per Person Max Benefit Amounts:

Medical Expense Benefit: ($1,000 for Plan 1 – $4,000 for Plan 7)

Outpatient covered expenses include physician charges, surgery, x-rays, reduction of fractures or other emergency first aid expenses incurred in a physician’s office, clinic, outpatient facility or ambulatory surgical center which are incurred within 28 days of the accident. Inpatient covered expenses include physician charges, hospital room & hospital billed services and supplies that are incurred within 28 calendar days of the accident.

Air & Ground Ambulance Benefit: ($2,500 for Plan 1 – $10,000 for Plan 7)

Pays benefits for covered expense for ground or air ambulance transportation (within 28 days of accident) due to an accidental injury.

Accidental Death Benefit: ($5,000 for Plan 1 – $10,000 for Plan 7)

For loss of life due to accidental injury (within 90 days of the accident).

Accidental Dismemberment Benefit: ($5,000 for Plan 1 – $20,000 for Plan 7)

Pays benefits if the Primary Insured suffers a specified dismemberment due to accidental injury within 90 days of the accident. This benefit applies only to the Primary Insured.

The total amount payable for all losses resulting from the same accident will not exceed the Maximum Dismemberment Benefit Per Accident. Dismemberment benefits are paid in lieu of other benefits if they exceed other benefits payable under the policy. If both an Accidental Death Benefit and an Accidental Dismemberment Benefit would otherwise be payable, benefits will be paid under the provision that would pay the most.

BENEFITS:

LUMP SUM FIRST DIAGNOSIS BENEFIT

SL will pay a lump sum benefit on first diagnosis of internal cancer or malignant melanoma. This benefit is paid only once for a covered person. Amounts are available from $10,000 to $50,000 in increments of $5,000. Maximum Benefit: $50,000

CHEMOTHERAPY, RADIATION & MEDICINES BENEFIT (Optional)

Pays up to a Monthly Maximum of either $2,500 or $5,000 for Actual Charges incurred for: Teleradiotherapy; Radioactive Isotopes Therapy; Chemotherapy; Chemotherapy Enhancer Drugs; Anti-Nausea; and Immunotherapy. Available for additional premium. Member’s Choice: $5,000/Month or $2,500/Month

If the covered person receives the above covered treatments at a government or charity hospital, the benefit payable is instead determined by the Government or Charity Hospital Benefit of $200 per day in lieu of the above benefit.

Issue ages: 40 to 85; Savers Advantage Home Health Care policy offered by Standard Life is designed for beneficiaries who prefer to seek and receive care in the comfort of their home versus entering a nursing home.

Benefits under this policy are payable regardless of any other coverage, including Medicare. Savers Advantage Home Health Care is available with two different levels of coverage: Classic and Deluxe. Extra Benefit Riders are also available with additional coverage options.

General benefit highlights for Classic and Deluxe are Home Health Care: Payment up to the daily maximum benefit, subject to eligibility conditions; Home Health Care Aide: Daily benefit, subject to eligibility conditions, for each day services are provided immediately following a hospital confinement of not less than three days; and Prescription Drug: per prescription benefit for both generic and brand scripts limited to the maximum benefit amount per policy year.

Extra Benefit Riders are Annual Physical Examinations: Payment per the schedule amount if policyholder has a physical exam performed by a physician more than 12 months after the rider effective date. Same benefit applies in each succeeding 12-month period; Accidental Death and Dismemberment: Benefits per schedule amount for an accidental death or an accidental bodily injury resulting in loss of finger, toe, hand, arm, foot, leg, or sight; Home Medical Equipment: Payment up to the maximum per benefit period when home medical equipment is required, specifically related to a sickness or injury for which Home Health Care benefits are paid. Covered home medical equipment limited to: mobility assistance, transfer aids, bathroom safety, home accommodations, and personal medical equipment.
