CVS Health acquired Aetna in November of 2018. Aetna still maintains its headquarters in Hartford, Connecticut, where they offer both group health insurance plans for employers and coverage for individuals and families. Their mission statement, published in 2018, reads:

Plan selections are Cancer only, Cancer and Heart Attack or Stroke along with an optional benefit: Intensive Care Unit Rider. Lump-sum benefits are paid directly to the policyholder or beneficiary.

Choose $1,000, $1,500, $2,000, $2,500 or $3,000 maximum benefit per policy year that covers dental, vision, and hearing per person. Plan deductible is $100 per policy year per person. Guaranteed acceptance and guaranteed renewable as long as premiums are paid on time.
Dental Coverage:
Class A: Preventative Care – No deductible, no waiting period. Coverage is 100% through a network provider. Exams and cleaning twice per year. Diagnostic examinations and x-rays included.
Class B: Basic Care – No waiting period, Plan pays 65% 1st year, 80% for years 2+, basic restorative services/oral surgery, periodontal services (non-surgical), fillings and non-surgical extractions, deductible applies.
Class C: Major Care – No waiting period, Plan pays 20% 1st year and 50% for years 2+, endodontics: includes root canals, periodontal surgery, bridges, crowns, and full or partial dentures, Implants: $1,500 lifetime maximum, deductible applies.
Orthodontics – 12-month waiting period, plan pays 50% for years 2+, $1,500 lifetime maximum, clear aligners ($750 lifetime maximum), deductible applies.
Vision Coverage: covered after 6 months and the deductible is met, plan pays 65% 1st year and 80% for years 2+, Pays up to $200 during any two policy years; eye examinations, eyeglasses, and contact lenses.
Hearing Coverage: covered after 12 months and the deductible is met, plan pays 80% for years 2+; Pays up to $500 during any one policy year; hearing examinations and hearing aids.

Home Health coverage allows for the freedom to choose where, when, and how you recover from an unexpected accident or illness. Policy benefits include the following: Daily hospital indemnity when confined in a hospital; home care indemnity pays when practitioner visits are received; registered nurse indemnity pays when hospital confined and at home; and when in-home services of a private duty graduate or registered nurse are medically necessary.
Two plan options:
Home Care Indemnity: pays a weekly benefit for in-home care
Daily Hospital Indemnity: pays a daily hospital indemnity amount if confined in a hospital, including observation stays in a hospital.

Medicare Advantage Plans are provided by private insurance companies approved by Medicare. These plans are also known as Medicare Part C. Medicare Advantage Plans are built to help individuals age actively. They often include prescription drug coverage and extra benefits. The costs may differ from the costs paid with Original Medicare. Here are some of the main differences.
After choosing a Medicare Advantage Plan, the beneficiary will continue to pay a monthly Part B premium to Medicare and sometimes pay a premium to the company that provides the plan. But there are some Medicare Advantage Plans that don’t charge an additional premium.
While Original Medicare has standard deductibles, they can vary among Medicare Advantage Plans. And some plans don’t require a deductible at all.
Individuals may pay a smaller copayment – a set amount you pay for care – with Medicare Advantage for a doctor visit than they would under Original Medicare.
Aetna Plan offerings and considerations may vary by plan and by geographies. Provider participation varies by state.
Insurance carriers don’t offer Medicare Advantage Plans in all states or all counties in a state.
