Ambulance Benefit
Ambulance transportation must occur within 180 days following the occurrence of the most recent Covered Specified Health Event.
Limit: Twice per occurrence of Covered Specified Health Event.
No lifetime maximum.
Transportation Benefit
Pays transportation costs when prescribed medical treatment that must be provided in a Hospital or medical facility that is located outside of a 75-mile radius of the residence. This benefit is limited to $1,500 per occurrence of a Covered Specified Health Event. Must be within 180 days of Covered Specified Health Event.
No lifetime maximum.
Lodging Benefit
Actual charges incurred for the Covered Person or any one adult member of the immediate family when a covered person receives prescribed special medical treatment at a Hospital or medical facility that is located outside of a 75-mile radius of the residence of the Covered Person within 180 days of Covered Specified Health Event. Not payable for lodging occurring more than 24 hours before or after special medical treatment.
No lifetime maximum.
Waiver of Premium Benefit (Does not apply in NJ)
If the Named Insured becomes Totally Disabled for 60 days as a result of a Covered Specified Health Event, ManhattanLife Assurance Company of America/The Manhattan Life Insurance Company will waive the premiums that fall due for the remainder of that person’s Total Disability. (Disability must occur prior to age 60. Premiums must continue to be paid for 60 days after commencement of disability. Benefit does not apply to spouse and children.)

Benefits include: DAILY INPATIENT HOSPITAL CONFINEMENT BENEFIT (per hospital admission) If member is confined in a hospital as a resident inpatient. Pays the daily inpatient benefit member selects (maximum of 10 days) (in ME, TX and UT, 31 days) per hospital confinement. In FL, payable for first 20 days of confinement then $10/$20 for next 11 days – depending on benefit level selected. This benefit is not payable for the treatment of Mental/Nervous disorders and substance abuse (in UT, or substance disorder).
HOSPITAL ADMISSION BENEFIT (1 per year) (in UT, 1 per each period of confinement) If member is admitted to a hospital as a resident inpatient. Pays the Hospital Admission Benefit selected.
DOCTOR OFFICE VISIT (2 per year)
Optional Benefits:
OUTPATIENT SURGERY BENEFIT (2 per year) For surgical services rendered in an Ambulatory Surgical Center or Outpatient Hospital Facility, pays the amount selected for outpatient surgery.
EMERGENCY ACCIDENT BENEFIT (4 per year) (FL maximum 2 per year) If member sustains an injury which requires emergency care by a physician in an emergency room or urgent care facility, pays the amount per emergency treatment. The treatment must be received within 72 hours of the injury. In FL, this benefit is payable only if member is confined as an inpatient within 24 hours of emergency treatment.

ManhattanLife’s Home Health Care plan allows members the flexibility and financial support needed to recover at home. This plan, underwritten by Standard Life, offers home health care benefits, home health care aide, prescription drug benefit, restoration of benefits, and extra benefit riders.
Plan features and benefits are:
Home Health Care Benefits – Daily max benefit of $150-$450 for skilled nursing, general nursing, physical therapy, speech pathology, occupational therapy, chemotherapy, enterostomal therapy, respiration therapy and medical social services.
Home Health Care Aide – Daily benefit for each day member requires services immediately following a hospital confinement for a minimum of three days.
Prescription Drug Benefit – Per prescription benefit of $10/Generic or $25/Brand.
