The legacy began on May 29, 1850, with the formation of The Manhattan Life Insurance Company. Manhattan Life introduced the incontestable clauses in life insurance contracts before any other company. Manhattan issued the first ever group life insurance contract as well as one of the first life policies to a woman.

Plan Information

Issue ages 18–85.

  • Two policy options with family coverage available.
  • Choose your dentist. No networks.
  • Policy year maximum: $1,000 or $1,500.
  • Waiting period: 12 months on major services.
  • Guaranteed renewable for life.

Dental Coverage:

  • Preventive services include semi-annual exams, cleaning, and x-rays.
  • No waiting period on preventive services.
  • Basic services include x-ray (other than “full mouth”), fillings, and extractions.
  • No waiting period on basic services.
  • Major services include bridges, crowns, full dentures or partials, full mouth extractions, and root canals.

Rates are tiered at individual only and family rates include a maximum of three children. Additional children beyond the maximum number are charged the age 3-17 rate per person. Not available in all states.

This is a limited benefit insurance policy for dental, vision, and hearing expenses. Policy is guaranteed issue, guaranteed renewable for life in most states, and is subject to company’s right to change premiums. Vision and hearing coverage are available as riders in some state(s).

Vision coverage:

  • Includes basic eye exam, eye refraction, and the cost of eyeglasses or contact lenses.
  • Waiting period is 6 months on eyeglasses and contact lenses.

Hearing coverage:

  • Includes exam, hearing aid, and necessary repairs or supplies.
  • Waiting period is 12 months on new hearing aids and existing hearing aid repairs.

  • Pays up to three lump sum benefits (four with the Recurrence Benefit).
  • Pays regardless of additional coverage.
  • Types of critical events covered: heart attack, stroke, coma, major third-degree burns, major human organ transplant, coronary artery bypass surgery, paralysis, end-stage renal failure and cancer.
  • All benefits paid directly to member.

Included in all 4 plans:

Ambulance Benefit

Ambulance transportation must occur within 180 days following the occurrence of the most recent Covered Specified Health Event.

Limit: Twice per occurrence of Covered Specified Health Event.

No lifetime maximum.

Transportation Benefit

Pays transportation costs when prescribed medical treatment that must be provided in a hospital or medical facility that is located outside of a 75-mile radius of the residence. This benefit is limited to $1,500 per occurrence of a Covered Specified Health Event. Must be within 180 days of Covered Specified Health Event.

No lifetime maximum.

Lodging Benefit

Actual charges incurred for the Covered Person or any one adult member of the immediate family when a covered person receives prescribed special medical treatment at a hospital or medical facility that is located outside of a 75-mile radius of the residence of the Covered Person within 180 days of Covered Specified Health Event. Not payable for lodging occurring more than 24 hours before or after special medical treatment.

No lifetime maximum.

Waiver of Premium Benefit (Does not apply in NJ)

If the Named Insured becomes Totally Disabled for 60 days as a result of a Covered Specified Health Event, ManhattanLife Assurance Company of America/The Manhattan Life Insurance Company will waive the premiums that fall due for the remainder of that person’s Total Disability. (Disability must occur prior to age 60. Premiums must continue to be paid for 60 days after the commencement of disability. Benefit does not apply to spouse and children.)

  • Pays directly to member, unless benefits assigned to provider(s).
  • Choice of benefits and premiums.
  • Pays in addition to all other insurance.
  • No deductibles.
  • No networks.

Benefits include: DAILY INPATIENT HOSPITAL CONFINEMENT BENEFIT (per hospital admission) If member is confined in a hospital as a resident inpatient. Pays the daily inpatient benefit member selects (maximum of 10 days) (in ME, TX and UT, 31 days) per hospital confinement. In FL, payable for first 20 days of confinement then $10/$20 for next 11 days – depending on benefit level selected. This benefit is not payable for the treatment of Mental/Nervous disorders and substance abuse (in UT, or substance disorder).

HOSPITAL ADMISSION BENEFIT (1 per year) (in UT, 1 per each period of confinement) If member is admitted to a hospital as a resident inpatient. Pays the Hospital Admission Benefit selected.

DOCTOR OFFICE VISIT (2 per year)

Optional Benefits:

OUTPATIENT SURGERY BENEFIT (2 per year) For surgical services rendered in an Ambulatory Surgical Center or Outpatient Hospital Facility, pays the amount selected for outpatient surgery.

EMERGENCY ACCIDENT BENEFIT (4 per year) (FL maximum 2 per year) If member sustains an injury which requires emergency care by a physician in an emergency room or urgent care facility, pays the amount per emergency treatment. The treatment must be received within 72 hours of the injury. In FL, this benefit is payable only if member is confined as an inpatient within 24 hours of emergency treatment.
