In 1935 the Equitable Mutual Aid Protective Society was launched. In that same year, Equitable entered the insurance marketplace when benevolent societies were placed under the authority of the insurance commissioner. Cancer

Equitable CancerCare+ is a cancer plan that can be used in addition to other insurance; and the benefits are paid directly to the policyholder or beneficiary, to be used however needed.  

First Occurrence is a one-time cash benefit of $1,000; after the original diagnosis of cancer, not including skin. Diagnosis made during first 60 days after the policy effective date is not covered.

Overnight hospital stays coverage includes hospital stays, drugs and diagnostic tests, doctor visits, and private nurse. In- or out-of-hospital coverage includes surgery, anesthesia benefit, blood, radiation therapy, chemotherapy, self-administered drugs, anti-nausea medication, and inpatient hospice care, ambulance and travel.

Outpatient coverage includes medical equipment, therapy provided by a licensed therapist, care facility benefit when confined in a skilled nursing or extended care facility, and home health care visits. Prosthesis coverage benefits for surgically implanted breast prosthesis and non-surgical breast prosthesis. Optional benefits include return of premium on death benefit and lapsed policy.

Equitable Heritage Protector Final Expense

Heritage Protector is primarily a final expense life insurance plan to help pay funeral costs, and cover healthcare expenses not covered by traditional health insurance, including Medicare.

Issue ages 50 through 85, benefits ranging from $2,000 to $30,000 based on plan selection. Benefits never decrease, premiums never increase; plan benefits are paid out in one lump sum.

Heritage Protector Level is first day coverage and full benefit is payable whenever death occurs. Underwriting rate classes are preferred and standard.

Heritage Protector Graded pays all premiums plus 5% interest to the beneficiary, if death occurs during the first 12 months. In the 2nd year, all premiums plus 10% will be paid. In the 3rd year, 75% of the selected benefit is payable. After 36 months, the full benefit is payable. If death is the result of an accident, the full benefit is payable from the first day of coverage.

Equitable EquiCash Hospital Indemnity Plan

EquiCash Hospital Indemnity plan is an alternative to help pay the increasing out-of-pocket expenses that insurance does not cover.

Plans offered are Silver, Gold, and Platinum, issue ages are 50 to 85. All EquiCash hospital indemnity plans include a daily hospital indemnity benefit, durable medical equipment, ambulance, emergency room benefit, physician benefit, and an optional lump sum cancer rider for an additional premium.

Medicare Supplement

Select from Plan A, F, G or N; Plans are available pre-65 due to disability in some states. Household or Joint Discount is available; state discount guidelines and rules may vary.

If applying under Open Enrollment based solely off Part B eligibility then proof of eligibility is required prior to policy issuance. Some states have other rules in place for guaranteed issue.   

Equitable offers three methods for submitting and completing an application: paper, express, or online express. Paper application can be submitted either by faxing or by uploading via agent portal. Express application is completed, from start to finish, over the phone with a specialist. To complete an Online Express application log on via agent portal. Access the support drop-down menu, click on Online Express application. Complete the application in full; submit application coupled with any required state specific forms. Once an application is submitted, an underwriter is assigned, and will contact the applicant to complete the interview.

All applications require a valid signature in order to be processed. Express and Online Express applications require a verbal signature. POA signatures are not accepted on Express and Online Express applications. Regular applications require a physical signature. POA signatures can be accepted only for open enrollment and guaranteed issue cases. 

Short Term CareChoices

Issue ages 50 through 84. The age is the person’s age on the day the application is signed; if the applicant has a birthday prior to policy issuance the age at signing is not changed. Application date is the date the application is signed. The effective date may be requested up to 60 days after the application date. Applicants living in the same household are eligible for a 10% joint discount. A CareChoices policy must be issued to both applicants to qualify.   

Equitable CareChoices is a Flexible Short-Term Care Insurance Plan. Two plans are offered.

Facility Only Care Plan: pays benefits for care, services and supplies in all covered facilities including Assisted Living Facilities, Nursing Homes, Residential Care Facilities, Hospice Care Facilities, Alzheimer’s Facilities or Units. 

Room Reservation: if hospitalized while confined in an Assisted Living Facility or a Nursing Home, the cost of the Assisted Living Facility or Nursing Home room is paid, up to the daily benefit amount selected. These benefits are paid for up to 15 days each year. 

Alternate Care:  helps pay for care not normally covered by the policy if there are no covered facilities in the area or if the alternate care is more appropriate to handle the needs. Equitable must approve the alternative plan of care.

Hospice Care: helps pay for care, services and supplies for terminal illness treatment.

Restoration of Benefits: Equitable will restore the number of days used in the benefit period (up to a maximum of 2 times the number of days selected) when discharged from a Nursing Home or Assisted Living Facility, or no longer receive Home Care, and have not qualified for benefits for 180 consecutive days.

Comprehensive Care Plan: benefits include the same as in the Facility Only Care Plan PLUS Home Care. Home Care covers care, services and supplies from qualified home care providers: visits by Home care aides, Nutritional specialists, Nurses, Medical social workers, and Physical, occupational and speech therapists.

Related Home Care Charges include transportation to and from medical appointments, domestic cleaning and laundry services, food shopping, running errands, meal preparation and clean-up. 

Respite Care pays benefits for care, services and supplies, up to the daily benefit amount, to temporarily relieve an unpaid caregiver who normally provides care in the home or private residence. Respite care is limited to 10 calendar days. It covers: Assistance with activities of daily living, Personal supervision due to cognitive impairment, Maintenance of the home environment, including shopping, meal planning and preparation, and light housekeeping.

Simple Inflation Protection of 3% is optional.

Short Term Nursing Home Care

Equitable EquiCare Short-Stay Nursing Home

EquiCare Short-Stay Nursing Home plan is an affordable and practical solution for nursing care for periods of less than one year. This plan has been designed to provide coverage for short-term nursing home stays of up to 350 days.

Maximum coverage can be selected or the cost can be lower by adding an elimination period. Daily cash benefit from $50/day to $300/day (depending on age). Benefit period of 100 days, 150 days, 200 days, 250 days, 300 days or 350 days; and an elimination period of 0 days, 30 days or 90 days. You qualify for benefits if two or more of the six ADLs (Activities of Daily Living) cannot be performed or if you have cognitive impairment (such as Dementia or Alzheimer’s disease). The plan is guaranteed renewable as long as premiums are paid on time. EquiCare offers a 10% discount for couples who are both insured.
